Controlled Substances
Controlled Substances are medications with addictive potential. Their
use is controlled by the Drug Enforcement Agency.
I only prescribe controlled substances at patients visits, never over the
phone.
Patients who are taking controlled substances agree to the following:
- The patient will not abuse drugs.
- The patient agrees to the administration of a urine or plasma drug screen at any time.
This drug screen will be obtained within 24 hours of the date and time of the order.
- The patient will not obtain prescriptions for controlled substances from any
other doctor, with the exception of emergency room visits.
- The patient will not consume alcohol.
- Prescriptions will not be refilled early, even if the medication or
prescription is lost or stolen.
- The patient will take the medication strictly according to the instructions on
the prescription.
- The patient will not give the medication to any other person to take.
- The patient will not take the medications of any other person.
- The patient will keep all appointments with the doctor.
- The patient will not lie to the doctor.
- The patient will not engage in any criminal activity.
- The patient will see any specialists to whom the doctor refers the patient,
and obtain any tests ordered by the doctor.
